After the hard stretch of treatment, you still have a next visit.

Some conditions are not “over.” When the intensive therapy is done, we help you live with the condition well: medicines, movement, counseling, and a name to call. That is management — not being forgotten.

  1. 1 of 4 · When does management start?

    When you and your clinician agree the intensive course is complete.

    That is a medical decision in your chart — not something a website declares. After that, the plan may be medicines, walking, counseling, or other long-term supports.

    Your CareCoach still checks whether the scale, the bottles, and the walking shoes are actually in the house.

  2. 2 of 4 · What does a year of management look like?

    A written plan, a yearly visit, and regular check-ins.

    If you have a long-term condition, you get a year-long plan: what to watch, which sheet to keep, when to come back. A yearly wellness visit is a good time to reset it.

    You should always know the next appointment. If you do not, call (352) 565-7342 before you leave it to chance.

  3. 3 of 4 · What if I have several conditions?

    We plan for the whole person, not five separate clinics.

    High blood pressure, diabetes, weight, and pain often travel together here in Citrus. You should not have to be the messenger between four teams.

    People with several conditions may be asked to come more often — even weekly when that is what safety requires. Ask why, and what we hope to change.

  4. 4 of 4 · Is management ever finished?

    The next slot is the test.

    A good lab number is a win. You still need refills, a ride, and someone who knows your name. After a hospital stay, management starts again the same week you get home.

    Keep the fridge sheet. Bring the bottles. Call if the plan stopped fitting your life.
