Assessments — so we are not guessing.
That can mean blood work, pictures (imaging), a mood screen, a walking test, or simply lining up your bottles. We order what helps. We do not pile on tests you do not need.
If a ride or a copay is blocking a test, say so. Getting the test done is part of care.
2 of 4 · Then what?
Therapies that you can actually use.
Counseling, physical therapy, a procedure, a medicine, or equipment such as a walker or a home scale. Our pharmacy is there so the bottle can leave with you when you are eligible.
We will not print a dosing table on a flyer. Your clinician and pharmacist go over the label with you.
3 of 4 · And after I am “as good as I am going to get”?
Management — the long game.
Many conditions are not cured; they are lived with well. You still get a next visit, refills, and a name to call. That is not being “kept on the hook.” That is how people stay out of the ER.
Your year-long plan is how the doctor, nurse, pharmacist, and CareCoach share the same week.
4 of 4 · What if I was in the hospital?
We want you in the same week you get home.
Bring the discharge papers and every bottle, old and new. Someone should check on you within a couple of days. If nobody called, you call us: (352) 565-7342.
