1 of 4 · What do you want to know from me?
Whether we listened, offered choices, and followed up.
A short survey may ask if we asked for your ideas, helped you set a goal, and checked back. That is more useful than a star rating alone.
Your CareCoach — a person on your team — is often the one who asks. You can also tell the front desk or call (352) 565-7342.
2 of 4 · What should a good visit feel like?
On time enough, private, and in words you understand.
We work to keep waits short and to get you a next appointment within about two clinic days. You should have a quiet place to talk. You should hear the plan in English or Spanish, without a rush.
If something felt off — a long wait, a hallway conversation, a form you did not understand — tell us. Privacy training is part of how we work.
3 of 4 · What about counseling and mood?
Staying with care is part of a good experience.
If you are coming for counseling or medicine for mood, we want you to be able to keep coming — in person or by video when that fits. We may ask two brief mood questions. A hard answer means we connect you with a clinician, not a lecture.
If you need a person right now, call or text 988.
4 of 4 · Will my answers change anything?
Yes. That is why we ask.
We look at wait times, whether people come back, and whether you felt the visit worked. Your answers help us add hours, change how we greet you, and train the team.
You will not lose your sliding-fee discount or your appointment because you were honest.
